Sliding Fee Scale Program

Our sliding fee scale program is in place so that all clients seeking services at Child and Family Service are assured that they will be served regardless of ability to pay. This program is designed to provide discounted outpatient mental health services to those who have no means, or limited means, to pay for their mental health needs.

Clients who have been screened to be appropriate for services will not be refused for service because of lack of financial means to pay.

Child & Family Services will not discriminate on the basis of an individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity or any other characteristic that makes a client unique.

The basis for a client’s payment is determined by the client’s annual household income and number of family members in a household.

Proof of income will be expected before or at the time of the first appointment by one of the following methods:

  1. most recent three consecutive check stubs
  2. most recent 1040 (copy of page that reflects combined household income).

Payment is due at the time of service. Questions about the Fee Scale may be directed to our Billing Specialist, Alison Gonzales, at 989-790-7500, ext. 256.
